GENERAL SUMMARY

The Licensed Practical Nurse provides care to patients and is responsible for performing selected tasks under the direction of the Registered Nurse and within the framework of supportive and restorative care. The Licensed Practical Nurse is knowledgeable about and supportive of the primary nursing model.

PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS

Clinical Practice: Demonstrates/applies the skill necessary to implement the medical/nursing plan of care, nursing interventions and procedures necessary for the care of the patient under the direction of the Registered Nurse.

  • Provides direct patient care within the primary nursing model to ensure continuity of care for all patients within their caseload.

  • Delivers care to patient’s in a safe efficient manner maintaining the patient’s rights and dignity at all times.

  • Introduces self to patients and families as the patient’s nurse.

  • Establishes a positive relationship with the patient and family.

  • Provides care to patient’s based on the primary nurse’s plan of care.

  • Bases all nursing interventions on pathophysiological and psychosocial rationale.

  • Documents all nursing interventions and the patient’s response in the medical record for all assigned patients.

  • Communicates all changes in the patient’s condition immediately to the Registered Nurse.

  • Delivers care in a fiscally responsible manner.

  • Demonstrates essential knowledge of pharmacology/medications in administering drugs and monitoring the patient’s response.

  • Administers medications as determined by the unit leadership team.

  • Utilizes available resources within the hospital and community to assist in the care and treatment of patients.

  • Participates in patient/family teaching.

Collaboration: Demonstrates/applies the skills necessary to interact with health care team in regards to the implementation of the medical/nursing plan of care.

  • Collaborates consistently with the primary nurse in the care of patients.

  • Collaborates with the primary nurse to assist with the development of the patients care plan.

  • Implements the patients plan of care on all assigned patients.

  • Collaborates with the primary nurse in evaluating the outcomes of care for assigned patients.

  • Promotes cohesive work environment by looking beyond individual assignment to the needs of the unit as a whole.

  • Delegates responsibilities to others with an understanding of their roles, knowledge and capabilities.

  • Maintains a collaborative work relationship with peers and colleagues in order to create a positive work environment.

Climate: The Licensed Practical Nurse influences others by establishing a climate for optimal patient care, mutually supportive collegial relationships and professional development.

  • Holds self accountable for professional practice.

  • Presents self in a calm and professional manner.

  • Participates in performance improvement activities as requested, utilizing performance improvement principles to support and improve patient care.

  • Attends and actively participates in staff meetings. Accepts responsibility for reading staff meeting minutes when not able to attend.

  • Keeps current with changing practices and interventions in patient care.

  • Assumes responsibility for seeking out educational and professional opportunities for personal learning needs and growth.

  • Assumes responsibility for meeting mandatory education requirements and unit specific education and competencies.

JOB KNOWLEDGE AND SKILLS

Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging process, human development stages and cultural patterns in each step of the care process.

Demonstrates the ability to provide care for a designated group of patients according to unit guidelines and hospital policies, procedures and protocols.

AGE SPECIFIC COMPETENCIES

Has the skills and knowledge to provide care to the age groups of the population served including:

  • knowledge of growth and development
  • ability to interpret age specific data and response to care
  • provide age appropriate communication

EDUCATION

  • Graduate of an approved Licensed Practical Nurse Program

EXPERIENCE

  • Dependent upon unit requirements and needs

LICENSES

  • Must have a current Massachusetts Practical Nurse License

WORKING CONDITIONS/PHYSICAL DEMANDS

This position is located in Framingham, MA

  • Prolonged, extensive, or considerable standing/walking
  • Lifts, positions, pushes and/or transfer patients and equipment
  • Considerable reaching, stooping, bending, kneeling, crouching
  • Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc.
  • Regularly exposed to the risk of bloodborne diseases and other transmissible infections
  • Contact with patients under wide variety of circumstances
  • Subject to varying and unpredictable situations
  • Handle emergency and crisis situations
  • Subjected to irregular hours
  • May have contact with hazardous materials
