JOB DESCRIPTION:

• TITLE: Behavioral Health Case Manager
• PAY: $35.00-38.00 (per hour| max rate based on experience)
• BENEFITS: Medical-Dental-Vision & more (eligible day 1)
• HOURS: M-F | 40.00-HRs (weekly)
• OTHER: $5,000 Bonus (paid out quarterly over a year)
• SHIFTS: 8-HR shift | 8-5pm CST (no nights, no weekends, no OT)
• LOCATION REGION: State of Texas
• LOCATION TYPE: Remote (must reside in location area / region)

REQUIRED SKILLS:

• State of Texas LPC, or LCSW License ~ (No LPC-A, LMSW Licensures)
• 1+ years of Behavioral Health and/or Counseling Experience
• Foster Care and/or Pediatrics Experience (preferred)
• Managed Care Org. Experience (preferred)

POSITION PURPOSE: Provides clinical information and education for inbound calls and outbound calls triages behavioral health crisis calls. Uses established protocols and independent professional judgment. Documents assessment, plans, interventions and results.

JOB RESPONSIBILITIES & DUTIES:

• Patient assessments via ZOOM and Telephone
• Document calls and actions using standard computer software application
• Provide resource and education to non-licensed behavioral health staff
• Schedule and make follow up calls to provide care facilitation
• Promote recovery concepts and inspire hope
• Work with other internal and external agency personnel to meet callers' needs
• Provide professional assessments, interventions within scope of relevant licensure
• Provide information and behavioral health education to callers
• Other and more duties assigned
