The nurse resident will obtain these skills at proficiency level within 12 months of hire. The resident will be under the tutelage of a mentor during this time. The nurse resident will attend the classes assigned by this program and demonstrate proficiency in the content. At the completion of the residency program, the job title will change to Field RN.
Assesses, plans, develops and provides with the patient or responsible caregiver a home health plan of care for nursing and identifies patient needs for other disciplines and services. Coordinates and evaluates the plan of care with therapeutic, social service and home health aide services, other organizations and physicians for an assigned caseload. Instructs patient or responsible caregiver in nursing care, health guidance, health promotion and management of health problems as directed by the physician’s treatment plan, and consistent with Agency regulatory and professional standards.
