Title: Health Coach - BRC - Unite Here Health - BI - Full-Time, Days

Collaborates with the UHH clinic team to help actively engage a panel of approximately 500 patients and co-develop and facilitate the implementation of customized care plans.


Roles & Responsibilities:

1. Communicates with patients using a variety of methods, both in person (individually or in groups) and by telephone, text, or email

2. Implements a range of interventions through counseling, motivational interviewing, coaching, education, referral and advocacy to assist patients in gaining access to services and resources

3. Provides patient education on chronic disease management on which the Health Coach will be trained

4. Communicates care plans and pertinent information with the internal care team and other parties participating in the care of the patient, as necessary

5. Assists with the coordination of care across the care continuum (including home care, outpatient care, ER care, and hospital care) while maintaining strict patient confidentiality at all times

6. Serves as a positive example and representative of the organization both internally and externally

7. Helps to build and perpetuate a practice environment and culture of collaboration, respect, learning, flexibility and fun among all team members


Requirements:

Education:

Bachelors' Degree or 3 years relevant experience required.

Experience:

1-2 years of experience in a service environment required (demonstrated commitment to health and wellness a plus)

Licensing and Certification

Completion of a Medical Assistant certificate program from an accredited school is required; Certified Medical Assistant is preferred.

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $58661 - $60000 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
