Join our team and make a positive impact on the lives of others! We are looking for a Nurse Practitioner or Physician Assistant to join our telemedicine primary care practice to service the thousands of chronically ill patients we serve daily.

This role is remote with potential travel to our main office in Lake Success, NY and/or the FreedomCare hub closest to you. Based locally in NY would be a plus.

Department & Position Overview:

As we continue to expand our service offerings to our patients, Nurse Practitioners and Physician Assistants. The provider will lead the charge to world class patient experience and quality outcomes by leveraging technology and innovation to provide exceptional digital and at home clinical care.

Every Day You Will:

  • This position is remote and will primarily support New York state, but will expand to support states throughout the country
  • Provide direct patient care as a Physician Assistant or Nurse Practitioner
  • Conduct virtual exams and accurately document findings
  • Diagnose and treat illnesses
  • Communicate effectively with other providers through comprehensive charting and messaging
  • Leverage telehealth technology to improve health outcomes
  • Ensure quality patient care through remote patient monitoring and in-home clinical care
  • Identify and share opportunities to improve quality of care and efficiency
  • Ensure regulatory compliance with all applicable requirements

Ideal Candidate Will Possess:

  • Experience practicing as a PA or NP from an accredited medical school and medical board certification
  • Experience in Internal medicine , primary care, or geriatrics
  • 5+ years practicing as a clinician
  • Experience working with Medicaid and Medicare patient populations
  • Experience providing patient education
  • Commitment to addressing health disparities by striving to improve care, access, and service to those affected by social determinants of health
  • Strong technical skills with ability to help patients navigate technical difficulties during virtual visits
  • Solution-centric, creative thinker with an entrepreneurial attitude
  • All-star relationship builder with excellent communication skills
  • Passion for leveraging technology to transform healthcare in this country

Nice-to-Haves:

  • Located in downstate NY and open to one day per week in the clinic
