Qualified Intellectual Disability Professional (Behavioral Health Support Staff)
Full time
Salary: $47,800/yr



Responsibilities:
  • Write and update individual’s POC, programs, data sheets, corresponding informed consents and staff training sheets; prepares for and participates in Plan of Care/ IDT team meetings
  • Conducts training on individual programming for facility and day program staff
  • Completes ongoing required documentation for each individual served, such as monthly documentation and progress notes.
  • Ensures follow-up to recommendations for services, equipment, or programs; and ensures that adequate supports and assistive devices are present to promote independence.
  • Conduct observations of all shifts at each facility and at the day program; provides feedback and recommendations to supervisors and staff regarding program implementation / observations
  • Ensure Comprehensive Function Assessment’s (CFA) are completed and reviewed annually; update as individual’s needs change
  • Assists with all legal, licensing, regulatory, and certifications activities as needed. Monitors plan of correction for ongoing compliance.
  • Develops and maintains working relationships with all clients, families, funding sources and regulatory agencies, as appropriate, and monitors their satisfaction with services.
Qualifications:
  • Bachelor's degree in human services related field and one year related field experience in the Human Services Industry
  • Skilled Excellence in written and oral communications
  • Knowledge of State ICF guidelines and regulations preferred
  • Experience with Facilities State Licensing and Recertification Policies and Practices
  • In depth knowledge and experience in creating, implementing, and measuring Individual’s Plan of Care, Behavioral Support Plans, and Comprehensive Function Assessments’
  • Experience in Presentations for Staff training and Support Meetings
  • Other education, experience, training, licensure, or certification per state requirements or as required by applicable regulations.
  • Current driver's license, car registration, and auto insurance.
  • Strong attention to detail, organizational skills, and the ability to multi-task to meet deadlines.
  • Effective communication skills to create effective relationships with internal and external consumers.
  • A reliable, responsible attitude and a compassionate approach.
