Summary of Responsibilities:

The APRN is responsible for the general physical and psychiatric care of the clients. The APRN will be responsible for examining, evaluating, and treating clients including assessment, documentation, and prescribing. The APRNwill work closely with the RN’s and LPN’s as well as conferring with the Medical Director to ensure the highest degree of care for all clients.

Essential Duties

· Assess/examine clients including physical and psychiatric exams and taking client history.

· Prescribe needed medications

· Authorize treatments and educate clients on continued care

· Communicate with the Clinical team to ensure continuity of care for clients.

· Complete all documentation in a timely manner

· Administers medications and for any adverse reactions or side effects

· Confers with and assists the Medical Director.

· Makes appropriate referrals

· Assists with emergency situations and make immediate referrals.

· Provides documentation which reflects services for each patient in accordance with
licensure standards.

· Review and/or complete H&P’s, Psychiatric Evaluations, and lab work within expected
time frame

· Review and sign UDS orders

· Review and sign sick call that are completed by nursing staff

· Sign and review orders that are put in by nursing staff

· Send eRX prescriptions when needed

· Be available for on-call needs

· Maintains the confidential nature of all patients and facility activities.

· Maintains required Training & Continuing Education.

· Maintains Standards of Ethical Conduct.

· Complies with all UpHealth Policies and Procedures.

· Effectively communicates concerns about patient health to the Medical Director and
follows up as necessary.

· Under the supervision of the Medical Director, assists in educating patients about health
problems and medical/psychiatric care.

· Assists the treatment team in incorporating medical/psych issues into treatment
planning.

· Participate in regular meetings with Clinical team to discuss specific patient problems.

· Encourage efficient utilization of supplies and equipment to prevent waste.

· Maintains effective communication between outside medical services and the facility.

· Participates in Performance Improvement, Utilization Review, Fire & Disaster Committees,
and others as requested.

· Interacts with clients with compassion and respect.

· Attends in-service education programs as assigned to learn new forms, procedures,
developmental skills, etc.

· Has a good work habit: get things done in a timely, efficient manner with no excessive
absences.

· Functions well as a team member and contribute positively to the morale of staff.

· Performs additional duties, as deemed necessary by Medical Director or EVP of Nursing.

· Maintains up-to-date training in CPR, First Aid, Crisis Intervention, Infection Control, Fire
Safety, and other areas mandated by organizational policies.

· Communicates effectively with other staff members and demonstrates the ability to work
as a team member.

· Demonstrates flexibility and adaptability, responding positively to changing situations in
the work setting.

· Demonstrates objectivity and maturity under stress.

· Accepts and utilizes supervision, performs other assignments as requested by supervisor

· Addresses problems noted by supervisor.

· Demonstrates willingness to accept responsibility.

· Completes all tasks with concern for work quality.

· Adhere to all company policies and safety rules, guidelines & procedures.

Necessary Abilities:

· Knowledge of appropriate nursing assessment, diagnoses, & interventions with patients

· Knowledge of milieu and crisis management

· Ability to direct subordinates in the performance of their duties

· Ability to accurately administer medication and monitor side-effects

· Demonstrates Proficiency in Communication & Written skills

· Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws

Licensure/Certification:

· Current MN State APRN License

· Current MN APRL License with Dual Certification in PMHNP & FNP

· Current CPR Certification

Minimum Qualifications:

· A minimum of one (1) year experience in a psychiatric health-care facility treating co-
occurring, mentally ill, or chemically dependent patients, with understanding of co-
occurring disorders.

· All employees may be required to work before, during and/or immediately after a natural
or man-made disaster
