JOB SUMMARY:
This position requires expertise in the nursing process and uses critical thinking skills to plan and coordinate patient care. This role collaborates with other team members to identify Medicare patients appropriate for Initial and Annual Wellness Visits (AWV) and execution of guided processes to complete the AWV. Assists with the triage of patient calls and completion of other clinical tasks as needed. Assesses patient healthcare needs that require additional follow-up by the primary care provider and assists with scheduling appointments. Collaborates to facilitate completion of Maryland Primary Care Program goals. Provides knowledge of community resources to patients and connects them with resources. Serves as a patient advocate and assists in the identification and improvement of service delivery.


Qualifications

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES

Education & Training:

  • Current Maryland RN license required.
  • Completion of a Bachelor’s of Science degree in nursing preferred.
  • Strong knowledge of Epic EHR preferred.
  • Work Orientation & Experience: Two (2) years of nursing experience in primary care, community nursing, critical care, or home health required. Knowledge of population health principles preferred.

Skills & Abilities:


  • Demonstrated skill in a) clinical case management; b) performing complete biopsychosocial assessments and medical triage; c) effective critical thinking; d) written and oral communications; and e) age-appropriate interpersonal interactions. Demonstrated knowledge of a) payer (incl. Medicare and Medicaid) regulations and programs; b) general internal medicine topics and c) case management principles.
  • Demonstrated ability to a) communicate and collaborate effectively with both internal and external customers; b) assess, adapt, and calmly respond to changing and/or crisis environment; c) make independent decisions consistent with current policies, procedures, and ethical standards; d) prioritize work assignments and manage time effectively to complete duties; and e) develop a structured and organized process for monitoring active care plans.
  • Must have proficient computer skills, including Microsoft Office (specifically Word and Excel)
