JOB DESCRIPTION:

The Population Health Coordinator (formerly known as panel coordinator) is an integrated Patient Centered Medical Home (PCMH) position responsible for clinical patient panel management activities. Under the general supervision of the Community Health Team Manager, the population health coordinator works closely with associated primary care providers, practice staff and health team members to assist with patient self-management, panel management, data collection efforts, reporting and analysis. This position develops/produces reports utilizing the electronic health record (EHR), Excel, or other relevant reporting tools. By gathering and organizing patient data, the population health coordinator works to identify patients unmet needs, engage patients in care, and enhance ongoing communication between the patient and their care team. The goal of the population health management program is to facilitate high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment.

EDUCATION:

High School diploma with a minimum of 3 years’ experience in customer service or in a health care setting or Associates degree with 1 year experience in customer service or a health care setting. Experience in promoting healthcare behavior change is desirable. Certifications preferred (CHES, TTS, Lifestyle Coach, LNA).

EXPERIENCE:

Minimum of three years of medical office experience required. Prior use of electronic health records and other health care information systems desirable.
